What happens next?

SCHEDULING & INSURANCE CONFIRMATION

Our team will contact you to collect insurance plan information and determine if there are additional fees, such as copay or deductible. This process takes 24-48 hours.

CLINICAL INTAKE 

After booking with Minder, the patient completes an online intake form, covering their demographics, medical history, and reason for seeking an evaluation.

Testing packet mailED to you

A packet is mailed to the patient's home, which is used to record responses during some of the tests (with their phone on a supplied tripod so Minder can see too). 2-3 days before the evaluation a "tech check" is scheduled.

TELEHEALTH EVALUATION

On the evaluation day, the neuropsychologist greets the patient via Zoom and conducts a clinical interview, followed by cognitive testing by a psychometrist. This appointment is typically 4 hours in length, but can vary.

DETAILED RESULTS & FEEDBACK

1-2 weeks after the evaluation, the patient attends a feedback session via Zoom, and the neuropsychologist explains the findings and recommendations and answers questions. A written report is emailed following the feedback session.

If you need to reschedule, please do so at least 10 days prior to your appointment, otherwise a $199 cancellation fee applies.

1. Confirm Insurance Verification

Selecting insurance during booking does not complete verification. Please ensure full insurance details are provided to avoid delays.

2. Complete Intake Questionnaire

Please complete the intake form immediately so your neuropsychologist can prepare for your evaluation.

3. Test Zoom Access

Test your Zoom connection before your appointment.

4. Prepare Equipment & Private Space
  • Computer or tablet (minimum 10" screen)
  • Working camera and microphone
  • Smartphone with internet access
  • Quiet, private room
  • Flat surface and adequate lighting
5. Tech-Check Call

We will call you 3–4 days before your appointment for a mandatory TechCheck. This call typically takes 5–10 minutes.
